Our Community Nursing Care services are designed to support NDIS participants with complex health needs who require care from a qualified nurse. This service is available for participants with a disability-related health condition that requires regular monitoring, treatment, or clinical support.

  • Medication administration and monitoring
  • Diabetes management (including insulin administration)
  • Catheter care and continence management
  • Stoma care
  • Enteral feeding (PEG tube care)
  • Chronic disease support
  • Post-hospital care
  • Health assessments and reports

 

Who Delivers the Care?

Our services are provided by qualified Registered Nurses (RNs) or Enrolled Nurses (ENs) with experience in disability and community health. We work closely with participants, families, support coordinators, and other professionals to ensure safe, personalised care.

Where is the Care Provided?

We deliver community nursing care in the comfort of your own home, supported accommodation, or another community setting based on your needs.

Is It Funded by the NDIS?

Yes – Community Nursing Care can be funded under the Capacity Building – Improved Daily Living or Core Supports budget in your NDIS plan, depending on your needs.
