Helps you build the skills needed to connect with informal, community, and funded supports. It enables you to get the most out of your plan and pursue your goals.
- Support Coordination (Coordination of Supports)
Assists you in building the skills required to understand and use your NDIS plan. Your Support Coordinator will work with you to:
- Coordinate a mix of supports
- Maintain relationships
- Manage service delivery
- Live more independently
- Be actively included in your community
